Case Management Services

Placement Partners serves individuals in Hennepin and Ramsey County under the Community Access for Disability Inclusion (CADI) waiver, Brain Injury (BI) waiver, Alternative Care (AC) waiver, Elderly Waiver (EW), and Rule 185 Case Management. You must first complete an assessment by Hennepin or Ramsey County, then to be referred to our services if eligible. Please use the following info to reach out to Hennepin or Ramsey County.
